* Metabolic or endocrine disease
Exclusion Criteria:
* Known myopathy
* Systemic steroid use (\> 1 mg/kg hydrocortisone) or exogenous Cushing's syndrome
* Psychological, family, sociological or geographical conditions that could potentially limit compliance with the study protocol
