. positive for hepatitis B surface antigen (HBsAg) OR
. positive for HBV DNA
. Alanine aminotransferase (ALT) \>2x upper limit of normal (ULN)
. Total bilirubin \>1.5x ULN (isolated bilirubin \>1.5xULN is acceptable if bilirubin is fractionated and direct bilirubin \<35%)
