Inclusion Criteria: * Physician diagnosed CRS with surgical requirement for treatment Exclusion Criteria: * Subjects with physician-diagnosed: * cystic fibrosis, * vasculitis, * any type of nasal tumor * receiving ongoing immunosuppressant therapy
