Adapt the initial model through feedback from clinicians and their patients who will be asked to test the tool then complete interviews and surveys. 3. Test the model with clinicians and their patients who will be asked to use the tool for several weeks. 4. Evaluate the acceptability and feasibility of using the tool in a pilot trial through surveys and interviews.
